The Gut Microbiome

The gut microbiome is a complex and ever-changing community of microorganisms living in the gastrointestinal tract. It includes bacteria, viruses, and fungi. This complex ecosystem plays a crucial role in our health and well-being. Maintaining a healthy balance of microorganisms is essential for overall health.
The gut microbiome can be influenced by factors such as diet, lifestyle choices and medications such as proton pump inhibitors (PPI’s) and antibiotics.
Ali can work with you to help improve the health of your gut microbiome.
The low FODMAP diet
FODMAP is an acronym for Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols. These are types of carbohydrates and sugar alcohols that can be poorly absorbed in the small intestine. On reaching the large intestine they may be fermented by the bacteria in the colon. This can lead to symptoms such as gas, bloating, diarrhoea, and abdominal pain in some individuals.
The low FODMAP diet involves reducing or eliminating high FODMAP foods from your diet and then gradually reintroducing them to identify specific triggers for your symptoms. The diet isn’t a one-size-fits-all solution, and it may not be appropriate for everyone with digestive issues. Individuals who undertake the diet with the guidance of a registered dietitian tend to achieve better results.
Ali completed the low fodmap training course at King’s College, London. She is fully fodmap qualified and has a wealth of experience to help you properly implement the diet.

Small Intestinal Bacterial Overgrowth (SIBO)
SIBO occurs when there is an increase in the bacteria in the small intestine. This can lead to a variety of symptoms such as abdominal pain, bloating, gas, diarrhoea and/or constipation. It can interfere with the body’s ability to absorb essential nutrients from food such as Vitamin B12. Malabsorption of nutrients can cause fatigue.
There should be a relatively small number of bacteria in the small intestine compared to the large intestine (colon). However, various factors can disrupt this balance, leading to SIBO. Some common causes include:
• Impaired gastric motility can slow down the movement of food and waste through the intestines and contribute to SIBO. Numerous factors can cause this including diabetes and hypothyroidism.
• Structural abnormalities such as diverticula or strictures, can disrupt the normal flow of intestinal contents and create pockets where bacteria can accumulate and overgrow.
• Certain medications, such as proton pump inhibitors (PPIs) and antibiotics, can alter the gut environment and increase the risk of SIBO.
• Diets high in sugar, refined carbohydrates, and low in fibre can encourage bacterial overgrowth.
Diagnosing SIBO can be challenging because the symptoms can be like other gastrointestinal disorders such as irritable bowel syndrome. Breath tests are commonly used to diagnose SIBO.
SIBO is a complex condition that can have a significant impact on a person’s quality of life. Early diagnosis and appropriate treatment can help manage symptoms and prevent complications. Treating SIBO typically involves a multifaceted approach. Antibiotics may be used to reduce bacterial overgrowth and diets such as a low FODMAP diet can reduce symptoms by limiting the types of carbohydrates that feed the bacteria. Lifestyle modification, probiotics and elemental diets are some of the other strategies used to help treat SIBO.

Intestinal Methanogenic Overgrowth (IMO)
IMO is an acronym for Intestinal Methanogenic Overgrowth also known as methane predominant small intestinal bacterial overgrowth, is a subtype of SIBO. It is characterized by an overgrowth of methane-producing bacteria in the small intestine. Overproduction of methane in the gut can result in various digestive symptoms such as abdominal pain and cramping, bloating, belching and flatulence. IMO is typically associated with constipation as methane can slow down intestinal motility.
As with other forms of SIBO, the treatment of methane predominant SIBO involves a multifaceted approach by a team of healthcare professionals. Antibiotics may be used to reduce bacterial overgrowth. Diets such as a low FODMAP diet can reduce symptoms by limiting the types of carbohydrates that feed the bacteria. Lifestyle modification and prokinetic medications which improve gut motility can also be of benefit.
